The
SUCCESS Independent Living and Aftercare program offers access to a
wide variety of services to youth in foster care or who have been discharged
from Children, Youth and Families.
The SUCCESS program is unique because it offers clients an opportunity
to gain independent living skills which will better prepare them for
the future. The program has been developed to address the issues of
youth who have been in foster care for an extended period of time, such
as dropping out of school, unplanned parenthood, homelessness, depression,
and suicide.
The program is geared towards youth in foster care between the ages
of 16 to 18, and young adults voluntarily in care between the ages of
18 to 21, and young adults who have "aged out" of foster care
after the age of 18.
Services
- Assessment
of each client's strengths and needs in order to provide further information
and training.

Development of individual transition plan, based on the assessment,
which includes the client's interests and goals, as well as the expectations
for fulfilling the program.
- On-going
case management.

Overseeing housing stability.

Job readiness skills.

Enrollment and advocacy in local school districts.

Behavior management / behavior modification programs.

Development of age appropriate social and emotional skills.
