Responsibilities:

  • Contact all referrals to Independent Living and Aftercare (ILA) Program as assigned by Manager through outreach, home visits, and contact with family members including foster families to successfully enroll youth in the ILA Program.
  • Maintain enrollment of youth in ILA Program through various means including outreach, home visits and contact with family members including foster families.
  • Develop individualized Independent Living Plan with youth and significant others when appropriate that identifies goals and objectives relative to each independent living domain and manage the delivery of services to ensure success in the attainment of competency in each domain.
  • Participate in meetings of ILA Program team members to discuss program progress, to discuss client participation and progress, and to develop program capacity and effectiveness.
  • Responsible for achievement of program goals and objectives and for monitoring client progress relative to individualized goals and objectives.
  • Responsible for delivery of quality program as measured through various means including funder expectations, client satisfaction, achievement of client goals, and achievement of program goals and objectives.
  • Responsible for management of assigned client records, completion of service documentation and completion of required reports.
  • Demonstrate an understanding of and respect for the cultural diversity of consumers, volunteers, staff, and other stakeholders throughout the provision of services.

Knowledge, Skills and Experiences:

  • Bachelor Degree in Social Work or other human service field and a minimum of two years experience working with youth and families preferred.
  • High School Diploma with a minimum of five years experience working with youth and families required.
  • Demonstrated computer knowledge of Excel, Outlook, Internet and Word/Windows environment.
  • Ownership of a reliable vehicle, valid PA driver’s license and proof of insurance.  Acts 33 & 34 and FBI clearance required.  Must be 21 years of age.
